Fever
This is a flushed face, tongue out, sweaty expression on his face, and it feels very hot, like the feeling of being in the sun for a long time. It can be used to express that the body has a fever, or is sick and weak, feeling too hot, thirsty for drinking water, or even eating something too spicy.
